What should I expect at my first visit?
IMPORTANT
Please be sure to reach out to your previous dentist to have your records transferred. We do require a full mouth series of x-rays (or equivalent) for your initial exam. It is your responsibility to ensure your records are sent from your previous dental office. Records can be forwarded to xrays@silversfamilydental.com.
Once you schedule your first appointment you should receive a link to our new patient forms. We kindly ask that you complete the requested forms prior to your appointment time. If you are unable to complete the new patient forms prior to your appointment, we ask that you please arrive 15 minutes prior to your appointment time to complete the forms in our office.
